- Notice of forms received will be sent to email of the centre, manager and coordinator.
- This will be a proof of participation of centre for the assessment year.
- Update new patients and reporting patient’s outcome. Please DO NOT wait until year end. To facilitate VERIFICATION OF INFORMATION, your checking of your centre’s patient list OF RESPECTIVE CENTRE(S) are available and ACCESSIBLE on-line.
